Job Responsibilities: As a Home Health Physical Therapist, you will be responsible for providing rehabilitative care and skilled nursing to clients in their homes. This will involve developing and implementing individualized care plans, using manual therapeutic exercises, gait training, balance, and other interventions, and educating clients and family members on necessary care and activities.
Requirements: To be successful in this role, you will need a current Physical Therapist license in the state, a minimum of one year of recent work experience as a Physical Therapist, and previous home health care experience welcomed but not required. Solid computer skills and prior experience with electronic medical records (EMR) are preferred.
