BAYADA Home Health Care is currently seeking a Physical Therapist (PT) to join our Sierra Vista Visits team.
Your Role and Responsibilities:
- Perform diagnostic tests and measurements, including mobility assessments, transfer status, stability, and balance testing.
- Develop and implement individualized care plans, incorporating manual therapeutic exercises, gait training, and other interventions.
- Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents.
- Continuously assess and revise the Physical Therapy - PT care plan to ensure optimal patient outcomes.
- Educate clients, family members, or representatives on rehabilitative care and activities necessary for independent living.
- Accurately document observations, interventions, and evaluations related to patient care management and services provided.
