We are looking for a highly skilled Occupational Therapist to become an integral part of our home health care team at BAYADA Home Health Care.

The selected candidate will collaborate with a multidisciplinary team to provide in-home rehabilitation services, focusing on delivering comprehensive care that meets the unique needs of each client.
Responsibilities and Qualifications:
- Evaluate patients in senior living communities or group homes for adults with intellectual disabilities, developing personalized plans of care.
- Collaborate with home health therapy partners to ensure smooth transitions from home health to outpatient services, maintaining continuity of care.
- Educate clients, families, and caregivers on developed care plans, empowering them to enhance functional independence.
- Perform assessments, treatments, and discharge planning for patients using an outpatient EMR platform.
Requirements:
- A current New Jersey Occupational Therapist license.
- Prior home care experience is beneficial but not mandatory.
- A minimum of one year of recent experience as an Occupational Therapist.
- Completion of an occupational therapy program accredited by the Joint Committee on Allied Health Education and Accreditation.
- Proven ability to work independently and manage time efficiently.
- Strong communication, organizational, and problem-solving skills.
- Proficiency in computer applications, particularly electronic medical records (EMR).
