BAYADA Home Health Care, a leading provider of home health care services, is seeking an experienced Occupational Therapist to join our team in West Topsham, Vermont. About the Role
- We are looking for a skilled Occupational Therapist to work with adult and geriatric patients on a per-visit basis in homes throughout Randolph, Topsham, and Chelsea.
- You will be an integral member of our multidisciplinary healthcare team, working closely with nurses and other therapists to provide rehabilitative care and skilled nursing to clients.
Key Responsibilities
- Conduct thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
- Develop individualized intervention plans to achieve favorable outcomes, taking into account cognitive, psychological, and motor aspects of performance.
- Assess equipment needs and environmental adaptations to enhance patient outcomes.
- Educate and instruct patients, family members, or representatives on rehabilitative care and activities necessary to promote health, safety, and independent living.
Requirements
- A minimum of one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
