Job Responsibilities:
- Assess clients' physical needs and develop individualized care plans.
- Educate clients and their families on safe and effective self-care techniques.
- Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions.
Requirements:
- A current Vermont Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Strong interpersonal, organizational, and problem-solving skills, with excellent communication and documentation abilities.
