BayCare Clinic - Green Bay Eye Specialists is looking for a Patient Care Coordinator to join our team in Green Bay, WI. We're looking for a compassionate, thoughtful candidate who wants to contribute to a positive experience for all our patients. The typical hours for this role are Monday-Friday from 8 : 00am - 5 : 00pm. No Holidays! No Weekends!
The Clinic :
BayCare Clinic is a successful, dynamic group comprised of 20 specialties. We are a physician-owned entity with joint ownership in Aurora BayCare Medical Center, a 167-bed hospital located in Green Bay, WI, offering us a built-in referral base. Each of our specialty practices have clinical autonomy and are financially sound.

The Practice :
We are a team of dedicated Ophthalmologists and Optometrists who exist to meet the ophthalmic needs of the people of Northeastern Wisconsin and the Upper Peninsula of Michigan. BayCare Clinic Eye Specialists offers more specialty services than any other provider in northeastern Wisconsin and we are the largest Ophthalmology practice north of Milwaukee, with five locations in northeastern Wisconsin. We have a highly trained, collaborative team of ophthalmologists, most of whom specialize in one aspect of eye health. We have cornea, glaucoma, retina, pediatric and oculoplastic specialists.

The Job :
- Processes, completes, and forwards co management forms.
- Call patients to confirm surgery arrival time at 48 hours in advance.
- Ensure co management letter is complete, correct and sent to co manage provider within 24 hours of surgery.
- Monitors and facilitates appointment scheduling for surgery and procedure follow up, to include online patient requests.
- Serves as resource to staff regarding patient scheduling issues and co management procedure questions.
- Performs general office duties, to include faxing and answering phone.
- Functions as liaison with billing department regarding co management questions.
- Serves as point of contact for referring offices regarding co management billing questions.
- Prioritizes tasks and works independently.
- Respects and maintains confidentiality.
- Works in a safe, compliant and ethical manner at all times.
