BayCare Health Systems is currently seeking an experienced Case Manager to join our outstanding and compassionate team in Hernando County! This is a field-based position that works throughout all of Hernando County.

  • Location : Behavioral Health Hernando, Hernando County
  • Status : Full-Time, Hourly
  • Shift Hours : Vary
  • Shift : Days 8 : 00 am- 6 : 00 pm
  • Weekend Work : Occasional
  • On Call : Yes

The Case Manager is responsible for :

  • Providing linkage, monitoring, planning and advocacy for the most appropriate mental health and community medical and social services.
  • Coordinating the service delivery system for patients.
  • Providing residents, faculty and community mental health practitioners with up-to-date available referral resources to provide continuity of care.
  • Performing other duties as assigned.

    • Required Education :

  • Bachelor's in Related Field, Human Services Preferred

    • Experience :

  • 1 year in Mental Health required

    • Required Certification / License :

  • BLS (Basic Life Support)
  • Driver's License
