BayCare Health Systems is currently seeking an experienced Case Manager to join our outstanding and compassionate team in Hillsborough County! This is a field-based position that works throughout all ofHillsborough County.
- Location: Northside Behavioral Health, HillsboroughCounty
- Status:Full-Time, Hourly
- Shift Hours:Vary
- Shift:8- hour shifts
- Weekend Work: No
- On Call: No
The Case Manager is responsible for:
- Providing linkage, monitoring, planning and advocacy for the most appropriate mental health and community medical and social services.
- Coordinating the service delivery system for patients.
- Providing residents, faculty and community mental health practitioners with up-to-date available referral resources to provide continuity of care.
- Performing other duties as assigned.
Required Education:
- Bachelor's in Related Field, Human Services Preferred
Experience:
- 1 year in Social Work required
Or - 1 year - Case Managementrequired
Required Certification/License:
- BLS (Basic Life Support)
- Driver's License
