To provide social work services including intake, assessments, investigations, on-going case management and court related activities for adults at risk and elderly individuals in Bayfield County.A. With consultation from Corporation Counsel and supervisor, performs Adult Protective Services including investigation, offer of services, filing of state reports and narratives, and coordination with law enforcement and judicial system as appropriate and in accordance with Chapters 51, 54 and 55 of the Wisconsin Statutes.
B. Coordinates with court, law enforcement, hospitals, service providers, nursing homes, families, attorneys and concerned citizens regarding concern for vulnerable adults and the need for additional supports and/or guardianships and protective placements.
C. Consults with Corporation Counsel regarding guardianships and protective placements.
D. Recruits and trains guardians.
E. Performs investigations for elder and adult at risk populations per state guidelines.
F. Maintains knowledge of resources available for target populations served.
G. Determines the range of needs through interviews, home visits, collateral contacts, etc.
H. Provides referrals to appropriate agencies and/or resources as necessary to meet consumer needs
I. Coordinates and provides case management services.
J. Performs initial determination for persons who may be subject to orders for protective placement and conducts annual Watts reviews as needed thereafter.
K. Monitors program expenditures for effectiveness and efficiency and ensures against over-expenditure of funds.
L. Provides intake for Aging and Disability Services Section.
M. Provides IA 1.67 nursing home referrals as needed.
N. Records and maintains complete and accurate documentation and records and performs other general office duties as required.
O. Provides informational presentations to other staff, professionals, and community groups related to department programs as appropriate.
P. Participates in Section and Department meetings, work-related committees, and trainings.
Q. Participates in supervision/consultation.
R. Performs other duties as assigned.A. Minimum of a four year social work degree from an accredited college preferred but will consider: sociology, psychology, criminal justice or other approved human services program.
B. Certified as a social worker in the state of Wisconsin or eligible for certification by holding a training certificate or temporary license and certified prior to the expiration of the certificate or temporary license _OR- licensed as a Registered Nurse..
C. Minimum of one year experience providing social work to target population(s).
D. Experience in computer use and keyboarding (50 wpm) including Microsoft Office products.
E. Valid Wisconsin driver's license and reliable transportation.

A. Ability to sit or stand for extended periods of time while performing duties.
B. Ability to drive.
C. Light lifting.
D. Ability to work out in the field and navigate environments that are a human health hazard and/or have limited physical access.

Salary : $60,133 - $65,291
