- Establish collaborative practice programs with oncologists to improve supportive care measures for patients undergoing chemotherapy.
- Develop and deliver educational services to pharmacy staff, nursing staff, and other healthcare professionals on oncology-related topics.
- Serve as a subject matter expert in oncology, providing drug information and reviewing Electronic Health Record care plans to ensure they align with current national guidelines.
- Round with healthcare teams to optimize therapy according to established national evidence-based guidelines and policy.
- Provide patient-specific medication education using the teach-back method for new oral oncolytics and intravenous chemotherapy.
- Review oncology care plans within the Electronic Health Record, including new plans and updates to existing plans.
- Identify and evaluate medication-related problems based on clinical presentation, history, and laboratory data.
- Serve as a preceptor to pharmacy residents and students.
- Participate in Oncology Service Line meetings to improve safety, efficacy, and quality of cancer care.
- Promote rational and cost-effective drug therapy, focusing on cost avoidance and savings within the Oncology Service Line.
