Job Description
The Oncology Clinical Pharmacy Specialist is involved in the care of cancer patients throughout their treatment. This role works collaboratively with other healthcare team members to ensure accurate and optimized treatment plans, as well as supportive care measures.
Responsibilities:
- Establish a collaborative practice program with oncologists to optimize supportive care measures for patients receiving chemotherapy on an inpatient and outpatient basis.
- Conduct staff development programs or educational services for pharmacy staff, nursing staff, oncologists, and other healthcare professionals.
- Serve as a subject matter expert in oncology, completing drug information questions for the oncology service line, reviewing Electronic Health Record care plans to ensure they are consistent with current national guidelines.
- Round with the Oncology Fellows, Nurse Practitioners, and Physicians to optimize therapy according to established national evidence-based guidelines and Bayhealth policy.
- Provide patient-specific medication education using the teach-back method for new oral oncolytics and intravenous chemotherapy as needed.
- Review oncology care plans within the Electronic Health Record, including new care plans requested by oncologists and ongoing reviews of existing care plans for updates and appropriateness.
- Identify and evaluate medication-related problems based on clinical presentation, available history, or laboratory data.
- Serve as a preceptor to pharmacy residents and students.
- Participate in Oncology Service Line meetings to improve safety, efficacy, and quality of cancer care at Bayhealth.
- Promote and demonstrate rational, cost-effective drug therapy, focusing on cost avoidance and savings due to medication use within the Oncology Service Line.
