General Summary:
The Community Health Worker is a frontline public health worker who is a trusted member with a close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. The Community Health Worker also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities including, but not limited to, outreach, community education, social support, and advocacy.
Responsibilities:
1. Provides direct services (e.g., provides basic screening tests, provides basic services, meets basic needs) and supports professional staff.
2. Provides ongoing follow-up, basic motivational interviewing and goal setting with patients/families.
3. Cultural mediation among individuals, communities, and health and social service systems (e.g., educating individuals and communities about how to use health and social service systems, building health literacy, and cross-cultural communication).
4. Provides culturally appropriate health education and information (e.g., conducts health promotion and disease prevention education that matches the linguistic and cultural needs of participants and/or communities).
5. Interview clients to obtain basic data and past medical history and determine income status/program eligibility.
6. Conducts outreach (e.g., follows up on health and social service encounters with individuals, families, and community groups; makes home visits to provide education, assessment, and social support; presents at local agencies and community events).
7. Knowledgeable about community resources appropriate to the needs of patients/families.
8. Sign patients up for My Chart, Unite Delaware. Record patient information in the EMR and other software by 24 hours after patient contact.
9. Performs administrative support duties, including typing, filing, answering phones, scheduling appointments, acting as receptionist, and related work as needed.
10. Attend regular staff meetings, trainings, and other meetings, as requested.
11. Maintain HIPPA compliance at all times.
12. All other duties as assigned within the scope and range of job responsibilities.
Required Education, Credential(s) and Experience:
- Education: High School Diploma or GED
Certificate Program
Successful completion of DE Community Health Worker Program.
- Credential(s): Driver License
- Driving Record Required: Per Bayhealth requirements for operating a vehicle, all staff must be at least 21 years of age, with a current class D driver’s license and no more and no greater than 7 points in a 24-month period and 2 red light violations in a 12-month period. See policy for further driver exclusions.
Preferred Education, Credential(s) and Experience:
- Education: Associate Degree in Business Administration or related field
- Credential(s): Healthcare/Behavioral Health certifications are helpful Experience: MT, Paramedic, or Certified Nursing Assistant preferred. Medical terminology and/or background preferred
