Job Description
At Applegate Recovery, a division of BayMark Health Services, we provide high-quality, patient-centered treatment for addiction to prescription medications and other opioids.
Key Responsibilities:
- Provide comprehensive care to patients with opioid use disorder, including medication-assisted treatment and counseling services.
- Conduct thorough assessments, develop individualized treatment plans, and monitor patient progress.
- Counsel patients on the disease of addiction, signs and symptoms of detoxification, and the importance of ongoing recovery support.
- Document patient information accurately and maintain confidentiality in accordance with HIPAA regulations.
- Collaborate with clinical coordinators and treatment center directors to ensure seamless patient care.
Requirements:
- Graduate of an accredited nurse practitioner program (AANP) with at least 6 months of Addiction Medicine and Primary Care experience required.
- Current licensure in the state of Louisiana.
- Drug rehabilitation training/knowledge required.
- Satisfactory drug screen and criminal background check.
