What are the responsibilities and job description for the DSP- Job Coach - Job Skills Trainer for Individuals with Disabilities position at Bayside Community Network?

We are currently seeking a full-time job coach to assist individuals with developmental and/or intellectual disabilities learn job skills at their new workplace and ensuring they receive the support they need during their employment; including transportation. The primary schedule will be Monday - Friday 8am until 4pm, however applicants must be flexible and may provide back-up coverage on evenings and weekends.
The primary duties are:
- Provide on-job training to individuals needing to learn specific job duties.
- Analyzes the job tasks and assists the individual with creating a routine.
- Attend yearly PCP meeting for individuals on their caseload & assists the individual achieve their goals according to their Person Centered Plan and documenting accordingly.
- Communicating effectively with employers to form productive working relationships and partnerships.
- Informing the program Manager, Job Developer and/or Employment Director of all concerns regarding employment by communicating in a timely manner.
- Ensuring that the individual is safe and healthy and that their needs are being met.
- Collects and records data into CIMS and completes any other employer documentation requested
- Supporting the individual in the community in a variety of settings based upon individuals’ choice. (ex. workplace, volunteering in the community, community outings)
- Assists with transportation to job site.
- The primary schedule is 8 am until 4 pm Monday – Friday. Applicants must be able to maintain a flexible schedule. Coverage could include the following: Visiting consumers at work after the hours of 4 pm. Applicants must also be able to provide backup coverage on weekends and evenings.
- Applicants must have a valid driver's license in good standing with at least 2 years of driving experience. Applicants must also pass local, state and federal background checks.
Job Type: Full-time
Pay: $18.95 - $19.33 per hour
