Job Summary:
The Nurse Care Manager works in conjunction with the CPRC team to provide education and support to clients who have or are at-risk for complex medical conditions in addition to mental health and or Substance-use disorders.

Key Responsibilities:
- Develop wellness and prevention initiatives.
- Facilitate health education groups by utilizing advanced knowledge of complex medical conditions.
- Participate in the initial treatment plan development for all of their Healthcare Home enrollees.
- Collaboratively develop a treatment plan of healthcare goals for individuals with co-occurring chronic diseases.
- Consult Community Support Specialists regarding patient identified health conditions.
- Manage the process of hospital and medical provider admissions and discharges for Healthcare Home patients.
- Train patients on complex health conditions regarding medical diagnosis, treatment, and medications.
- Implement, as required, DMH Net Health technology programs and initiatives such as Cyber-Access and metabolic screening.
- Monitor and report performance measures and outcomes using sophisticated information systems developed for nurses.
Education and/or Experience Qualifications:
- Active RN or LPN license
- Experience in direct patient care preferred
- Knowledgeable in chronic disease processes in children and adults
- Excellent written and verbal communication skills.
- Ability to implement new innovative systems.
- Ability to make decisions based on critical thinking and nursing process
