Requirements :

• Experience in community outreach, social work, or related fields
• Bilingual proficiency
• CERTIFICATION as a Community Health Worker (CHW)

Job Description : Primary Purpose :

The Community Resource Coordinator II supports community connection activities by linking members to essential community resources, addressing Social Determinants of Health (SDOH) barriers, and facilitating care management efforts. This role emphasizes cost-effective solutions, health education, and community engagement to promote member well-being.
