Our client, a well-established nonprofit organization in Brooklyn that provides services for individuals with developmental disabilities, is seeking a bilingual (Spanish) Intake Coordinator. This part-time, permanent position offers flexibility, with hours from 9 AM to 3 PM, or the option for full-time. Compensation is $24–$26 per hour, depending on experience.
Role Overview: The Intake Coordinator will engage with families seeking assistance, helping them apply for the appropriate programs they qualify for. This role involves guiding families through the application process, offering consistent follow-ups, and re-engaging those who have previously fallen out of the process. - Experience in case management, family advocacy, or social services is a plus but not required.
- Basic knowledge of Microsoft applications – efficiently tracks outreach and manages data.
- Detail-oriented – ensures accurate follow-ups and documentation.
- Problem solver – resourceful and solution-focused.
- Creative – thinks outside the box to engage families and develop outreach strategies.
- Goal-oriented – driven to meet and exceed targets.
- Previously employed at either Tri County, ACA or Care Design
Key Responsibilities:
- Develop and maintain client relationships on a monthly basis.
- Reconnect with families who previously inquired but did not move forward, working to build trust and renew interest.
- Conduct intake assessments to understand family needs and match them with appropriate care services.
- Maintain organized records of outreach efforts and client interactions.
- Work collaboratively with the care management team to facilitate seamless client onboarding.
- Act as an advocate for members, helping them identify which programs they need, verifying eligibility, and supporting them with the application process.
- Take detailed notes during client interactions to ensure accurate and thorough documentation.
