Our mission is to make compassionate mental health care accessible to our community, making Orange County the happiest and healthiest community in the United States with a world-class mental health care system.  Job Summary:The Case manager provides services and referrals to residents referred to by the Mobile Operations Team that works alongside the Police Department. This position is under direct supervision of the Mobile Operations Manager for the Huntington Beach, CA Mobile Team.  What You´ll Do:

• Demonstrates the Be Well mission, purpose, values, and beliefs in everyday language and contact with the internal and external stakeholders. 
• Provides a safe, effective, and efficient implementation of direct care in accordance with established policies, procedures, and standards of care. 
• Makes appropriate recommendations for living environments, therapy, psychiatrist visits, case management, and other services. 
• Provides services to all residents referred based on their needs and preference.  
• Documents and communicates interactions with members to other services team members as appropriate. 
• Maintain awareness of community resources and assist members served in the appropriate use of community resources. 
• Advocates for and accompany members when needed on long appointments, e.g., SSI, DMV, GR (General Relief), etc. 
• Help members to understand their legal status and rights. 
• Safeguard confidentiality; adhere to HIPAA policies. 
• Foster independence with members served. 
• Safeguards confidentiality of members served. 
• Provides transportation when needed and appropriate. 
• Must participate in a rotation schedule of after-hours on-call services based on program expectations. 
• Duties and responsibilities may be added, deleted, and/or changed at the discretion of management. 
• Maintain confidentiality and respect the boundaries of clients experiencing emotional distress. 
• Attend required department and clinic meetings and share in other responsibilities as relevant. 
• Other duties as assigned. 

  What You´ll Need:

• Experience in a mental health and/or dual diagnosis case management position (preferred).  
• Bi-lingual Spanish (preferred).
• Two years of case management experience (minimum). 
• Knowledge of Orange County resources (housing, substance abuse, mental health, shelters, etc.) 
• High School Graduation or G.E.D. 
• Demonstrated sensitivity to multicultural populations and issues is required.
• Valid and unrestricted California driver’s license with clean driving record with reliable transportation.
• Must be CPR and First Aid certified on the date of employment or within 60 days of employment and maintain current certification throughout employment.

• Ability to work effectively as a member of a multidisciplinary team. 
• Effective written and verbal communication. 
• Good computer/technology skills. 
• Ability to assess safety risks. 
• Ability to promote a positive, supportive recovery culture. 
• Ability to engage clients and hold hope in the face of adversity. 
• Ability to gain trust and build rapport with diverse populations. 
• Ability to be patient and mindful. 
• Knowledge of basic medical and psychiatric terminology.

Pay Range
