QUALIFICATIONS
1. Experience in managing the social service needs of the mentally ill to include but not limited to:
- family, group and individual psychotherapy with adults and/or geriatric patients
- discharge planning and case management of inpatient psychiatric patients
2. Minimum education required is a MSW (Masters Degree in Social Work) from an accredited school of social work.
3. Must be licensed to practice Social Work in the State of operation (LCSW), maintains current professional licensure and demonstrates proof to Human Resources annually.

SPECIFIC RESPONSIBILITIES AND DUTIES
1. Supervises all aspects of the inpatient and outpatient therapeutic clinical program provided by the hospital. Assures that therapy staff are competent and that the inpatient and outpatient clinical program is focused on meeting the needs of the patients served.
2. Provides and supervises those who provide, direct and indirect social work services in accordance with the physician’s plan of treatment.
3. Assesses and reassesses social and emotional factors in order to help the patient and family cope with problems in daily living to help them follow medical recommendations to maximize the client’s health condition.
4. Counsels and communicates to patients/families/primary caregivers issues concerning the facilitation of the medical plan of treatment.
5. Recognizes and utilizes community and family resources to assist the client in long-term plans for life in the community or to learn to live with the patient’s disability.
6. Develops and supervises those staff that develop, the plan of care with the multidisciplinary clinical team, physician, patient, and family to deal with psychosocial
difficulties that may interfere with the health and wellness continuum. Develops discharge plan in conjunction with multidisciplinary clinical team, patient and family/caregiver.
7. Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies, submits accurate documentation within mandated time frame. Assures clinical staff are documenting progress records in accordance with hospital and regulatory standards.
8. Keeps abreast of social work trends and knowledge, obtains required annual continuing education in-services.
9. Participates in the hospital’s QAPI program and is responsible for assuring that clinical standards, social services P&P and medical record documentation of all social service staff are adhered to. Ensures compliance with the hospital’s established QAPI plan and takes responsibility for aspects of the plan that he/she must measure, assess and show improvement.
10. Cooperates with appropriate staff to provide staff education when requested and arranged by the Director of Nursing and Administrator.
11. Attends and participates in weekly treatment team meetings and makes adjustments to treatment plan accordingly.
12. Completes psychosocial assessment within time frames established per policy.
13. Ensures family contact, if applicable, is made with and documented in 24 hours of admission.
14. Serves as a liaison in the community as needed.
15. Responsible for completeness of treatment plan and applicable updates during treatment team conference and upon discharge.
16. Adheres to hospital’s infection control and safety policies.
17. Participates in determining the qualifications, hiring, assessing competence and utilization of all employees within the therapy department of the hospital.
