POSITION SUMMARY - Part-time position available
Provides comprehensive case management services to older adults (60 ) experiencing mental health challenges and/or other needs specific to aging within the community. This role involves assessing needs, developing individualized plans of care, coordinating resources, providing support and advocacy, and monitoring client progress to promote recovery, independence, and improved quality of life.

This role involves assessing needs, developing individualized plans of care, coordinating resources, providing support and advocacy, and monitoring client progress to promote recovery, independence, and improved quality of life.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The essential functions include, but are not limited to the following:
- Provides direct support and advocacy to clients in accessing community resources, including but not limited to:
- Mental health services (therapy, psychiatry)
- Medical care
- Housing assistance
- Financial assistance (e.g., Social Security, Medicaid/Medicare)
- Transportation
- Home health care
- Meals on Wheels or other nutritional programs
- Social and recreational activities
- Collaborates and communicates effectively with other service providers, including physicians, therapists, family members, and community agencies.
- Educates clients and their families about mental health conditions, available resources, and coping strategies.
- Responds to referrals from community agencies, Older Adult Educator, and interagency contacts.
- Assists older adults experiencing a mental health-related crisis and help coordinate recommended treatment as needed (e.g., hospitalization).
- Completes required chart documentation in accordance with agency policy and guidelines.
- Maintains knowledge of the use of Statewide Mandated reporting for Adult Abuse and Neglect.
- Participates in the planning of the annual Older Adult and Caregivers Conference.
- Maintains required training and certifications.
- Maintains agency productivity standards.
- Maintains a valid drivers’ license and reliable transportation.
- Other Duties and/or responsibilities may be assigned by the supervisor.
MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
Education:
- Associate degree or prior work-related experience required.
- Bachelor’s degree in the Social Services field preferred.
- Active driver’s license and minimum state-required auto-insurance.
- CPR/First Aid Certification required.
Experience Required:
- 2 years in a similar related position or work with Older Adults required if no bachelor’s degree.
- If bachelor’s degree in social services field, 1 year of prior work related experience preferred.
Required Knowledge:
- Knowledge of behavioral health diagnoses and symptoms
- Knowledge of Older Adult populations.
- General knowledge of community resources and/or the willingness to research as needed.
Skills/Abilities:
- Excellent communication skills, both written and verbal.
- The ability to use and troubleshoot basic business equipment.
- The ability to interact with a wide range of individuals.
- Strong ability to multi-task with a high level of accuracy and timeliness in project output.
- The ability to maintain a high level of client confidentiality.
- Ability to problem solve quickly and prioritize tasks for optimum efficiency
