Job Summary:
The Older Youth Case Manager provides direct support towards 10 youth in the program. This Case Manager will assess each youth’s needs, develop an individualized service plan, and assist them in achieving independent living goals. The position involves case management, crisis intervention, life skills training, and coordination with other community resources.
Duties/Responsibilities:
- Case Management:
- Develop individualized service plans for each youth, outlining specific goals and steps to achieve independent living
- Monitor and evaluate the progress of youth in achieving their goals and adjusting plans as needed
- Provide one-on-one support to youth, offering guidance in areas such as education, employment, health, and budgeting
- Life Skills Coaching:
- Provide training and support in essential life skills, including but not limited to:
- Financial literacy (budgeting, saving, managing bills)
- Job readiness and career planning
- Household management (cooking, cleaning, laundry, etc.)
- Healthy relationships, conflict resolution, and communication skills
- Facilitate workshops or group sessions to build these skills in a supportive environment
- Provide training and support in essential life skills, including but not limited to:
- Crisis Intervention:
- Respond to and address crises or emergencies involving the youth, including housing instability, mental health, substance abuse, or interpersonal conflicts
- Ensure youth are connected to appropriate resources for short- and long-term solutions (Maintaining boundaries is especially important while dealing with clientele personal conflict)
- Resource Navigation:
- Assist youth in identifying and accessing community resources, including education, healthcare, employment, transportation, and housing opportunities
- Build and maintain relationships with local organizations, service providers, and employers to expand opportunities for youth
- Facilitate connections with social services, healthcare providers, and legal resources
- Monitoring & Documentation:
- Maintain accurate and up-to-date records for all youth in the program, including case notes, progress reports, and goal tracking
- Participating in initial and ongoing training related to data collection and reporting
- Regularly assess and report on the effectiveness of the program and make recommendations for improvements
- Maintain HIPAA policies and regulations
- Advocacy:
- Advocate for youth by connecting them with legal, educational, or social support services that promote their well-being and independence
- Ensure that youth understand their rights and responsibilities and assist them in making informed decisions
- Collaboration:
- Work closely with other team members (e.g., social workers, counselors, housing providers) to ensure comprehensive care and support for each youth
- Attend and contribute to team meetings, case conferences, and staff training sessions
Knowledge, Skills, and Abilities:
- Skills:
- Proficient in Microsoft Office Suite
- Strong communication and interpersonal skills
- Ability to build rapport with youth from diverse backgrounds and challenging life situations
- Crisis management and problem-solving abilities
- Ability to perform in a fast-paced environment
- Knowledge of community resources, social service agencies, and programs for youth
- Ability to work independently and manage a caseload effectively
- Certifications (if applicable):
- CPR/First Aid Certification (preferred)
- Valid driver’s license and reliable transportation (if required for travel)
Education and Experience:
- Education:
- Bachelor's degree in social work, Psychology, Sociology, or a related field (preferred). Alternatively, relevant experience may be considered.
- Experience:
- At least 2-5 years of experience working with at-risk youth, homeless populations, or individuals in foster care
- Data entry and recordkeeping.
- Experience in case management or social services is preferred
- Experienced in crisis management and problem solving
