- Coordinate patient admissions, discharges, and transfers to ensure timely and efficient care delivery.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
- Identify and mitigate potential barriers to care delivery, ensuring timely and effective resolution.
Requirements:
- Current state licensure as a Registered Nurse.
- BCLS certification required; ACLS certifications preferred.
- Minimum six months' Medical/Surgical experience in an acute care setting preferred.
