Job Summary:
Responsible for organizing, managing and maintaining the Nursing Facility’s medical records according to Company Policies, State and Federal Regulations, including HIPAA and HITECH; must communicate courteously and respectfully with co-workers, Residents and others
Qualifications:
Prefer B.S. Degree and/or two years experience in Medical Records position, preferably in long-term care, or experience as a Certified Nurse Aide or Licensed Nurse; must have basic computer skills, good oral and written communication skills; must pass drug screens and have a satisfactory Criminal History Background Check
Duties and Responsibilities:
- Obtains Residents’ medical records from other medical facilities; sets up medical records for new Residents and assists in the Admission process
- Obtains approval of Residents’ Certificates of Medical Need through state and local medical facilities
- Maintains a master index file for in-house and discharged Residents
- Collects information from nursing staff, therapists, physicians and other sources to keep records updated
- Maintains records as directed for budget and Medical Records supplies
- Provides information to the Business Office as needed to meet admission requirements
- Regularly monitors and audits medical records for completeness and accuracy according to Company Policy
- Informs Administrator and Director of Nursing of any deficiencies in documentation
- Develops and maintains a system to share all necessary information for Admissions, leave passes, discharges, hospitalizations and deaths to Department Heads and all affected personnel
- Participates, as requested, in monitoring MDS, CAAS, Care Plans, CQI Committee Meetings
- Participates in interdisciplinary Care Plan Meetings and in Continuous Quality Improvement (CQI) program
- Processes, transfers and discharges Residents according to company policy
- Closes medical records, records Date of Discharge and enters data into medical record and computer
- Complies with all company policies, state and federal regulations regarding Medical Records
- Prepares statistics, reports and records for state or federal agencies as directed by the Administrator
- Provides information to interdisciplinary meetings so that quality of care can be maintained
- Accurately registers and records Death Certificates with vital records registration according to law
