Position Summary : The Account Executive primary objective is to support its client s and healthcare provider services coordination and practices. This will be accomplished by collaborat ing with discharge planners, case managers, clients, referral sources, professional resources and families at designated facilities or home within an assigned territory to coordinate the initiation of appropriate health care service s. Th is position is responsible for overseeing the quality of care as the patient transitions through the healthcare s ystem , with a heavy emphasis on coordination of Remote Patient Monitoring . The Account Executive is responsible for establishing and developing new community and provider relationships and assure current relationships remain strong . This position provide s continued support t o Bedrock ’s customers , including medical practices and its providers , senior living communities , acute care hospital s and post-acute care centers .
