COMPANION PROVIDER POSITION
We are seeking enthusiastic caregivers who are willing to provide care to an individual who cannot live independently in their own community and doesn’t want to live in a communal setting. Host Home Caregivers share homes and lives with individuals with Developmental Disabilities (IDD). As a Live-In Caregiver you move into the client's home and provide them hands-on support, and ongoing daily care. You can help someone who has some daily challenges and make a difference in their life as they become part of your family. Caregiver will have their own living area in a furnished basement and access to kitchen and living areas.
About The Individual
I’m searching for a host home provider to move into my 3-bedroom condo in Estes Park and provide services in my home. The condo is an exceptional living arrangement. I have a bedroom upstairs. There is another bedroom upstairs that can be used as a study or guest room. The kitchen and living room area are spacious and well-appointed with gas fireplace and modern appliances, including washer and dryer. There is a separate bedroom and bath suite downstairs with a large living area and a separate entrance. The monthly rent for this condominium is $400.
What People Like and Admire about me: I am kind, funny, friendly and social. I love to sing! I like to joke and am creative. I am flexible and always willing to learn and grow! I love to write.
What's Important to me: I enjoy watching movies, participating in choir, musicals and plays. I like swimming, bowling, whittling, Special Olympics and shopping. Following rules and being respectful are important to me, as well as having a job. I have a job at the YMCA of the Rockies being kept available for me; my team and I are searching for a job coach before I start. I like to spend time with my family and dog, Penny. I love poetry writing. It is important to me to have a healthy and active life. I participate in physical therapy, music therapy, church, meditation group and counseling sessions.
What's Important for me: I like to have support to maintain a healthy lifestyle and food choices. I use my forearm crutches, walker or wheelchair. I like to have verbal cues occasionally if needed. Being heard and having time to process my thoughts is helpful for me. I also like to sit in front of the car to prevent getting car sick.

TO QUALIFY FOR THIS AMAZING OPPORTUNITY
- Must have experience with providing personal care
- Must have experience working with individuals with intellectual and developmental disabilities
- Must have ability to use adaptive equipment for transfers and bathing, training provided
- Should have a high school or equivalent education (Required)
- Must have a valid Driver’s License
- Must complete training for Medication Administration, CPR/First Aid and maintain ongoing training as deemed necessary
- No pets or full-time children
- Good sense of humor, patience, and a dedicated personality a plus
- Companions must be willing to be onboarded by Support, Inc and pass the basic requirements
- The above includes completing a full application and passing background and reference checks
Caregivers Duties And Responsibilities
- Provide personal care
- Assist with oral hygiene care, meal preparation
- Provide a variety of recreational activities to enhance social development
- Include client in family life and activities
- Accompany client to supermarkets, museums, parks, dances, recreation centers, etc.
- Provide transportation as needed
- Assist with medical care, including appointments and administering medications
- Provide documentation of progress notes and medical records
- Provide training on life learning skills
- Pay $400 monthly in rent to the landlord

- Assistance in finding Respite and home care Aid(s)
