Our office is seeking a dedicated and experienced Community Based Rehabilitation Service Provider/Case Manager. Applicant must have a bachelor's degree in a Human Services related field. Those with a Social Work degree must be licensed in the State of Idaho. Those not licensed must be have certification or working towards their certification with the Psychiatric Rehabilitation Association. Must be able to pass a criminal history background check. CBRS/Case Management is an in-home and community based service where staff work one-on-one with consumers to teach them life skills.

As a Social Worker/Case Manager, you will be instrumental in providing comprehensive case management services to individuals and families in need. Reporting to the Program Director, your role focuses on assessing client needs, developing intervention plans, and facilitating access to resources. With a Bachelor's degree in Human Services or Social Work, you will utilize core skills in social work and premium skills in crisis intervention and addiction counseling.
