- Provide direct psycho-social services to nursing home residents through comprehensive evaluation, diagnosis, and ongoing treatment.
- Develop and implement individualized treatment plans to address residents' needs.
- Collaborate with Interdisciplinary teams to ensure seamless care transitions and quality outcomes.
**Requirements:**
- LCSW License
- Masters from an accredited University plus 1-2 years' experience
- Strong communication and collaboration skills
