Position Summary
Provide follow-up behavioral health care coordination services to clients or others such as parents/guardians concerned about an individual. Follow-up eligibility is based on contact center procedures and specific program requirements. The coordinator works with the caller and/or client to monitor symptoms and functioning while coordinating post-crisis care with existing providers or linking the client to new services.
Essential Functions:
Provide follow-up behavioral health care coordination services to clients or others such as parents/guardians concerned about an individual. Follow-up eligibility is based on contact center procedures and specific program requirements. The coordinator works with the caller and/or client to monitor symptoms and functioning while coordinating post-crisis care with existing providers or linking the client to new services.
Essential Functions:
- Increase the client’s knowledge of and skills about effective strategies to decrease health disparities and stigma related to LGBTQI and Black youth seeking behavioral health services.
- Provide telephonic outreach and engagement to increase access to family support and evidence based and culturally appropriate mental health services.
- Aid in connecting youth clients to evidence-based and culturally appropriate mental health services to youth, under 21, with or at-risk of SED/SMI and their families that are disproportionately experiencing homelessness or at risk of homelessness with a focus on LGBTQI youth and youth of color.
- Assist participants in developing treatment plans.
- Complete regular communication with referral sources/guardians regarding progress, transition planning, and pertinent clinical issues and documentation.
- Participate in staffing to assure continuity of care.
- Make or assist in outside referral of issues not able to be addressed within the treatment milieu. Assist in scheduling treatment and arranging transportation.
- Represent the agency in a professional manner.
- Identifies cases requiring follow-up and assists in the assignment of those cases to a coordinator.
- Maintains contact with individuals to coordinate care, help access community resources, continue to plan for client’s safety, and to aid the client in improving level of functioning post-crisis.
- Ongoing assessment of mental health symptomology, including suicidal thoughts/behaviors or other lethality risk factors.
- Ensures all follow-up and outcome documentation is completed for all cases.
- Participate in regular team meetings and individual supervision for case consultations.

- Provides training and ongoing support to clinical staff on follow-up procedures.
- Provides feedback on follow-up systems and works with Follow-Up Programs manager to enhance follow-up procedures.
- Provides protocol support, live call assistance and case consultation to clinical staff.
- Reviews cases as part of Quality Program and to provide supportive feedback to clinical staff.
- Participates in community activities such as system of care committees and resource events to raise awareness of services.
- Will handle inbound call duties across follow-up accounts as needed.
- Perform various other duties and functions as required or assigned within area of expertise or scope of the position.
Education, Experience, Competencies and Skills Required:
- Required Master’s Degree in Social Work, Psychology, Counseling
- Licensed or pursuing license preferred.
- One year of clinical experience preferred.
- Someone who has personal experiences with or strong understandings of multiple marginalized identities.
- Position uses various platforms such as on-site face to face visits, conference calls, messaging, email for communication with team.
- Must be flexible with shift, willing to move hours, work weekends and overtime as needed to ensure care coordination and service referral is completed.
- Strong service coordination and team building skills.
- Applied Suicide Intervention Skills Training-Required-can be coordinated upon hire.
- Ability to learn and adhere to multiple protocols.
- Excellent customer service skills.
- Strong verbal, written and interpersonal skills.

Tuesday - Saturday 11:00am - 7:30pm CST
