General Description:

Under the supervision of the Immunization Program Manager, Medical Authority, working under the general direction of Licensed Clinical Staff such as an APRN, RN, or LVN and in collaboration with the rest of the Immunization team, the coordinator will be responsible for providing moderately complex identification, outreach, education, data entry, and follow-up for the families of children who are consented to participate in the statewide immunization registry, ImmTrac2, but who do not have complete immunization histories in the registry. As well as outreach to TVFC Providers who use ImmTrac2 including technical assistance and training. TVFC education to providers to assist in the proper use of the electronic registry will be the main focus of the position but clinic work will also be a requirement.

This is an essential duty position in relation to designated Public Health Emergency and local disasters. During public health emergencies and local disasters, this position will be required to take on additional tasks and work additional hours to meet the needs of the community as it relates to the Health Department. In the event of a public health emergency, you shall report to work if ordered to do so by either of the following officials or positions: (1) Public Health District Director, (2) County Judge/Emergency Management Coordinator, (3) PHEP Program Manager.

Hours: This is not a remote work position. Daily in-office attendance is required 7 am – 4:30 pm, Monday – Thursday and 7 am-11 am on Fridays. Some weekend and evening work is required

Essential duties and responsibilities may include, but are not limited to, the following:

Immunization Information Systems (75%)

· Promotes the ImmTrac2 registry to adults and parents/guardians of children, educating them on the importance of consenting and having all their immunizations recorded in ImmTrac2.

· Performs mailings based on the ImmTrac2 client list.

· Responds to phone calls for information or questions regarding immunizations.

· Identifies individuals with incomplete immunization histories and updates vaccine records in ImmTrac2.

· Conducts outreach to organizations in our jurisdiction regarding missing vaccinations for children and adults for whom consent had been granted but who do not have complete immunization records.

· Networks with organizations in our jurisdiction to provide education, training and technical assistance to promote the effective use of the Texas immunization registry by organizations.

· Presents ImmTrac2 information to providers, schools, daycare facilities, and other community groups.

· Provides training, education and technical assistance to newly enrolled organizations, such as searching clients in the registry, reporting vaccines to the registry, conducting reminder and recall activities and update providers on enhancements and new features for ImmTrac2.

· Conduct outreach to existing registry users within the jurisdiction who have not logged into the registry in the last 90 days (3 months).

· Conduct outreach to organizations that have an expired site agreement within our jurisdiction within six months of their site agreement expiring and maintain documentation of all education and assistance provided.

· Conducts Texas Immunization Registry Immtrac2 Support Reviews with providers.

· Downloads and reviews scheduled reports from ImmTrac2 (monthly); 18-year-old target client report, Texas Immunization Provider Summary (TIPS), Provider Activity Report (PAR) and the Consent Acceptance Rate Evaluation Report (CARE).

· Conducts required outreach activities, including those focused on 18-year-olds in high school or college to increase the consent rate and the understanding that ImmTrac2 registry is for all age groups.

· Performs data entry into an automated records system; verifies and updates client demographic information and immunization histories.

· Coordinates outreach to promote the ImmTrac2 program; occasionally requires working after-hours or on weekends.

· Tracks outreach activities, education provided as required for quarterly reports.

· Identifies, recruits and trains new ImmTrac2 registry users.

· Attends and participates in meetings and conferences as required.

· Performs other duties as assigned by supervisor.

· During public health emergencies (and local disasters), will be required to take on additional tasks and work additional hours to meet the needs of the community as it relates to the Health Department.

Clinical Service Delivery (15%)

· Conduct Health District Immunization Clinics weekly at the Killeen/Temple office sites.

· Provide office site clinic services during evenings/weekends.

· Perform accurate assessment of immunization records.

· Utilize ACIP recommendations for vaccine usage

· Enters data accurately into EMR (electronic medical record)

· Perform Monthly reminder/recall activities

· Performs responsibilities in a manner that promotes quality patient care and customer service/satisfaction.

· Effectively participates in planning and preparing for patient visits ensuring appropriate schedules and clinic specific resources are available.

· Rooms patients consistent with clinic needs while determining and documenting chief complaint and reason for visit, vital signs, medication list, allergies, and other information as required by clinic. Keeps patients informed of delays and progress of clinical appointments.

· As required and competency validated, performs appropriate specimen collection, lab draws, injections, equipment cleaning/reprocessing and other requested procedures.

· Prepares and stocks rooms (daily/weekly) and clinic supplies in advance of clinic visit as well as cleaning rooms appropriately between patients.

· Schedules patients for follow up appointments or referrals and performs routine clerical duties for team and staff to include photocopying, scanning, and faxing.

· Performs other duties as assigned by the Clinic/Program manager or Licensed staff.

Vaccine Management-Primary or backup vaccine coordinator (10%)

· Ensures all vaccines are managed according to ACIP recommendations.

· Maintains the appropriate vaccine stock level supply per TVFC/ASN manual guidelines.

· Ensures wasted, expired, or unaccounted for vaccines do not exceed 5%.

· Performs vaccine inventory counts in Killeen/Temple offices.

· Performs monthly reporting to State database (VAOS)

· Performs all VLR (vaccine loss reports)/Wasted and expired product reports and shipping.

Qualifications

MINIMUM QUALIFICATIONS

· High School Diploma or equivalent

· Valid TX driver’s license preferred

· Reliable mode of transportation

· Current CPR certification (if applicable)

· Current valid certification as a Medical Assistant (preferred)

· One year of experience as a medical assistant in a healthcare setting (if applicable)

bilingual preferred

Knowledge or the ability to quickly learn:

· Applicable local, state and Federal statutes, rules, codes and regulations governing ImmTrac2 program operations.

· Immunization program criteria.

· Records management.

· Department policies, procedures and standing delegation orders (SDO) covering specific areas of assignment.

· Knowledge of the Universal Precautions/Bloodborne Pathogens.

· Principles and practices of project management.

· Ability to record observations completely, accurately, legibly, and concisely.

· Knowledge of fundamental principles of human behavior and an appreciation of the effects of stress upon individuals and groups.

· Knowledge of need to refer specific situations to licensed staff for disposition.

Skilled in:

· Communicating effectively, both orally and in writing. Communicating successfully with individuals, and the general public in offsite settings

· Establishing and maintaining cooperative working relationships with managers, supervisors, employees, external public agencies and organizations, vaccine providers, clients and the general public.

· Knowledge and ability to perform procedures appropriate for the scope of the medical assistant.

· Using initiative, discretion and decision making within established procedures, guidelines and rules.

· Ability to administer vaccines to all age groups and perform phlebotomy.

· Ability to observe general physical condition of patients to detect signs of abnormalities.

· Defining problems, collecting data, establishing facts and drawing valid conclusions.

· Providing effective customer service.

· Ability to interact effectively and in a supportive manner with people of all backgrounds.

· Ability to work efficiently and effectively.

· Ability to work well under pressure, multi-task and handle stress well.

· Operating a personal computer and various software applications.

SPECIAL CONSIDERATIONS/REQUIREMENTS/RECOMMENDATIONS:

· Ability to lift clinic supplies, educational materials and machines.

· Ability to stand, sit, and drive for prolonged periods is required

· Increased risk for exposure to and possible transmission of vaccine-preventable diseases and/or tuberculosis.

· Must have appropriate immunizations according to BCPHD Staff Member Immunization Policy.

WORKING ENVIRONMENT & PHYSICAL DEMANDS:
