Looking for hands-on work that makes a positive impact in people’s lives? This may be the position for you. The successful MH Case Manager will assess individual mental health, drug, and alcohol, and/or children’s needs of Shelter residents, and make referrals to appropriate services. The Case Manager provides supportive guidance and advocacy for the mental health needs of program participants, including crisis intervention; transport residents as needed to accomplish program goals; and design and instruct, arrange mental health and life skills workshops. Case Manager is responsible for preparing and maintaining required documentation and records, including client-centered goal plans, discharge plans, and aftercare records. This is a full-time salaried position.
Minimum requirements: bachelor’s degree in social science. Must be at least 21 years old, possess valid driver’s license, insurable driving record, and have reliable transportation. PA State Police background check, Act 33 and FBI clearances
