Position Summary : Bellefaire's Street Outreach Program is looking to add a Street Outreach Worker to the team. Staff are needed to go in the field and assess homeless youth's immediate needs as well as will need to be able to engage at-risk teens and their families.
Responsibilities Include :
- Perform community outreach and client assessment by preparing individual service plans under service teams
- Provide case management, service coordination, and ongoing aftercare services
- Conduct data collection and case documentation
- Represent the agency in the maintenance of relationships with other organizations in relation with the client and their families
