What are the responsibilities and job description for the Street Outreach Navigator position at Northeast Ohio Coalition for the Homeless?
To support unsheltered individuals/families in preparing for, securing, and retaining permanent housing, by assessing barriers to permanent housing, developing goals, objectives and plans that address the barriers, which includes obtaining and retaining employment and other income; to provide individual mental health services and monitor progress made toward achieving the goals, objectives and ISP’s; to complete file documentation and reports on results and outcomes. Performs other duties of a similar nature as may be required.
