We are looking for a compassionate Resident Care Coordinator to join our team! The Resident Care Coordinator is responsible for assisting the Resident Care Director and Mind & Memory Care Director in the successful operation of the resident care department, including hiring, training, scheduling and participating in all resident care associate disciplinary action. This position will be responsible for departmental compliance and will serve as a member of the community management team.
Responsibilities
Assures and maintains compliance with all resident care and state related regulations (e.g., medication management, introductory visits, skills checks, in-services, service plans, etc.)
Monitors overtime usage.
Schedules resident care staff for Assisted Living and Memory Care
Assists Resident Care Director (RCD) in developing an ongoing plan of care based on the identified physical and psychological problems of the resident including: the identified problems; resident specific goals and planned program interventions.
Assists Resident Care Director and Mind and Memory Care Director in developing a service plan according to the requirements outlined in the state regulations.
Assists Resident Care Director in accurately assessing, communicating, and documenting residents’ status.
Assists Resident Care Director and Mind and Memory Director in communication of Care Staff levels and needs; including Lead Aide and Medication Aide needs.
Observes and reports any significant changes in resident behavior and health to the Resident Care Director
Assists RCD in accurately completing a follow-up process for all incidents and accidents.
Assists RCD in effectively handling follow-up issues with residents and families.
Co-Facilitates RCA meetings.
Requirements
Must be a Certified Nursing Assistant or Home Health Aid with valid Massachusetts certification.
Must have a minimum of two years of experience working with elders as well as demonstrated supervisory and management skills.
Valid driver’s license
Physically able to move at least 50 lbs. without assistance.
