Status: Weekday (flexible with no minimum time requirements)
Setting: Assisted living, independent living
EMR: Net Health
Qualifications:
Active PA PT License in good standing
CPR certification
Current PPD/TB test
Covid vaccination or approved exemption

Responsibilities Include:
- Providing effective and engaging rehabilitative services
- Recording patient responses to treatments and reports outcomes of treatment through proper documentation in Net Health and facility documentation systems
- Establishing relationships with your patients to keep them engaged and positive about their treatment4.
- Providing guidance to family members and facility staff
- Conducting yourself in a kind, courteous and professional manner to all patients, family members, facility staff and fellow team members
- Keeping a positive, teamwork approach
