Responsibilities:
- Develop and implement community based day service plans developed through a person-centered planning process.
- Insure the completion of annual assessments, support strategies, quarterly reports and other documentation as required by regulations.
- Act as a liaison with community organizations to develop volunteer activities, internships and employment as well as to attend events, visit places of interest
