Support individuals within the ABI / MFP program who live in their own home, assisting with personal care, activities of daily living, individual support, companionship, homemaker, and community habilitation.
Duties include :
- Develop rapport with individuals supported and foster their personal growth
- Monitor and report consumer's progress, challenges and status
- Work individually with any program participants, which may include assistance / supervision with mobility, recreational activities or other needs Skills / Abilities :
