Best Life Mental Health Services, LLC. is seeking a new role for our team! We are a multidisciplinary group practice that specializes in mental health, with an emphasis on our mission: to provide inclusive, research-driven mental health and related services for all, with an emphasis on engaging those who have met barriers in receiving quality care.

The Targeted Case Manager (TCM) will provide case management services, including assessing clients' needs, conducting psychosocial evaluations, developing individual treatment plans and maintaining daily progress notes. They will provide education and support to clients and family members as well as make appropriate referrals to outside community partners. At Best Life, we want to meet multiple areas of a client's needs in attaining self-sufficiency and achieving more stable and maintained mental health gains. The Targeted Case Manager will maintain a billable caseload of 25 patients maximum. They will monitor all aspects of client cases and prepare and maintain client records within the electronic medical record provided by Best Life Mental Health Services, LLC.
Per Kentucky law, Best Life Mental Health Services, LLC. requires all TCM applicants to meet the following criteria:
- A minimum of one full year of working within a mental health setting that includes diagnostic presentations of mental illness
- Have completed the courses for TCM certification in the state of Kentucky.
- Maintain certification and required continuing education to remain certified.
- Participate in supervision with TCM Lead.
- Provide face-to-face services that are in-person twice per month around Jefferson County, minimally and more depending on client acuity.
- Be able to obtain consents and communicate/collaborate with outside providers.
- Complete prior authorizations to obtain approval for services.
- Create an individualized treatment plan for each client.
- Provide adequate documentation for clinical and billing purposes.
- Education: Bachelor's degree in related field as defined in 908KAR 2:260.
