Responsibilities

- Coordinate and facilitate access to medical services, addiction counseling, and other necessary resources.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
- Maintain accurate medical records and documentation in compliance with HIPAA regulations.
- Collaborate with healthcare providers, social workers, and community resources to support patient recovery and well-being.
- Provide diagnostic evaluations and ongoing support for patients facing behavioral health issues.
- Advocate for patients' rights and needs within the healthcare system.
Requirements
- Bachelor's degree in Social Work, Nursing, Psychology, or a related field; relevant certifications in case management preferred.
- Strong knowledge of HIPAA regulations and commitment to maintaining patient confidentiality.
- Experience in addiction counseling or hospital medicine is highly desirable.
- Proven ability to provide high-quality patient care with empathy and professionalism.
