Family Success Network Coach
FLSA Status: Non-Exempt
Definition and Primary Objective: Provides ongoing supportive consultations and flexible case management services for up to 20 families, delivered either virtually or through a combination of phone calls, emails, virtual or in-person through individual meetings. Offers parent education and connects families to resources needed to strengthen their self-sufficiency and capacity to meet the need of their children.

Collaborates with: Family Success Network Manager (UWGC)

Minimum Education, Experience and Other Skill Requirements:
- Minimum Associates degree in Social Work, Early Childhood Education (CDA), or related field from an accredited college or university; Bachelor’s degree preferred. Will accept education and work experience that meets requirements for Career Pathways Level (CPL) 3 as outlined by Ohio’s Early Childhood Professional Development Network
- Experience building trusted relationships and communicating with diverse groups and individuals developing mutual understanding, respect and empowerment.
- Experience providing home visiting, case management or peer support services to families or children preferred.
- Knowledge of accessing community resources required.
- Experience with family-driven problem solving, solution-based outcomes and collection of screening tools.
- Demonstrated experience working within community-based systems to remove barriers for families seeking services, knowledge of accessing resources.
- Valid driver’s license and reliable transportation required.
Primary Responsibilities:
- Responsible for meeting all administrative expectations including but not limited to timely and accurate completion of program documentation and family/child records.
- Submit regular programmatic and fiscal reports to the FSN Program Manager and to other identified partners requesting data.
- Complete intake paperwork to include required program/funder forms and inventories.
- Prepares curriculum, Individual Prevention Plans, FSN forms and inventories, screenings and community resource referrals for home visits.
Internal and External Collaboration
- Attend meetings amongst collaborative members.
- Collaborate with evaluation staff to facilitate and support the completion of the updated needs and resource assessment for the project and to adhere to evaluator guidance on evaluation/assessment administration.
- Develop relationships with local social service partners and community resources to build and maintain a professional, responsive relationship with key community partners.
- Consult/collaborate with other provider agencies and systems on behalf of the client and family. Serve as point of contact for the county of service.
- Assist families in gaining access to essential community resources.
Direct Client Service
- Provide short-term, intensive, family driven, in-home services. Offer a series of one to two home visits each month and access to the fully array of the six pillars (family coaching, parent education, basic life skills, financial literacy, service referrals, and concrete supports) of service delivery.
- Develop a prevention services plan, outlining the family driven goals and utilizing evidence-based programming to enhance the families’ protective factors and increase the family’s stability.
- Administer and review universal screenings, assess family strengths and provide financial literacy training, and/or concrete supports. Tools may include: the Adverse Childhood Experiences (ACEs) Survey the Child Trauma Assessment Checklist (CTAC); UNCOPE (substance abuse screening); and a mental health screening tool.
Work Environment:
Exposed to a combination of office, home and community-based environments. Exposed to children with behavior problems. May be exposed to shouting and clients who may be verbally and physically aggressive. May be exposed to blood or vomit and bodily fluids.
Position schedule is Monday through Friday – to include evening hours however, schedule may vary to accommodate meeting with families in the community as needed. May include weekends.

Regularly required to sit, stand, walk, bend and lift objects of up to 20 pounds. Expect significant walking in a community setting and standing during home visits, may require frequent climbing and descending stairs.
