Job Description:
*Manager of Care Management (RN or MSW) at our St Jude Medical Center in Fullerton, CA.
*This is a full-time position that will lead and manage the Care Management Team of RNs and Social Workers.
Required qualifications:
*Bachelor’s degree in nursing or master's degree in social work.
*3 years of Experience in an ambulatory or acute care setting.
*Strong project management and operations experience.
Preferred qualifications:
*Master's degree in nursing or health administration.
*Case Management Certification (CCM or ACM) upon hire
*3 years of Management experience in a Healthcare setting.
Candidate Details:
*2 to 5 years experience
*Seniority Level - Mid-Senior
*Minimum Education - Bachelor's Degree
*Willingness to Travel - Occasionally
