We are partnering with one of the top healthcare facilities in Southern California to find a dedicated and experienced Manager of Care Management. This full-time leadership role is based in Fullerton, CA, and will involve overseeing a skilled team of registered nurses (RNs) and social workers, ensuring optimal care management for patients.
Job Overview: As the Manager of Care Management, you will be responsible for the daily operations and clinical oversight of case management services within a well-regarded medical center. This includes managing case managers and coordinating support staff to ensure patients receive exceptional care throughout their healthcare journey, from diagnosis to treatment and recovery. The role involves advocating for patients, promoting health optimization, and facilitating access to necessary resources.
Key Responsibilities:
- Lead the care management team, including RNs and social workers, in daily operations and clinical practices
- Ensure case managers are effectively advocating for patients, achieving optimal health outcomes, and utilizing resources efficiently
- Oversee case management services across the continuum of care, ensuring that patients receive the best possible care and attention
- Collaborate with hospital leadership to enhance patient care protocols, case management processes, and overall service delivery
- Support the achievement of Magnet Designation by ensuring high standards of care and service delivery

Compensation:
- Base Salary: $113,000 to $178,000, depending on experience
- Comprehensive benefits package
- Full-time position, with no bonus or commission opportunities

Job Requirements
Qualifications Required:
- Bachelor’s degree in Nursing (BSN) or Master’s degree in Social Work (MSW)
- A minimum of 3 years’ experience in an ambulatory or acute care setting
- At least 1 year of experience in healthcare leadership, managing a team or department
- Strong project management and operational skills
Preferred Qualifications:
- Master’s degree in Nursing or Healthcare Administration
- Case Management Certification (CCM or ACM) upon hire
- Experience working in a Magnet hospital environment
