Requirements
KNOWLEDGE, EDUCATION, SKILLS AND ABILITIES REQUIRED:
- Education – An associate (A.A.) degree with 2 years of experience, or a Bachelor’s degree with 1 years of experience in a clinical, medical, or mental health setting
- 1 years’ of experience working with Behavioral Health and/or Substance Use disorder patients is preferred
- Proficiency in the use of EMR, experience in medical chart review, including MS Excel, MS Word, PowerPoint, and internet navigational skills; and highly developed interpersonal and communication skills.
- Bilingual Preferred. Proficiency in English and Spanish or Chinese, both written and spoken language
- Experience working in medical settings and interacting collaboratively with medical teams. Possess a working knowledge of HIPAA regulations; FQHC experience preferred
- Excellent communication and organizational skills with professional demenor.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Provide patients and their families with guidance and support in understanding mental health and substance use treatment.
- Identify patients with opioid /substance abuse disorder who wish to be in treatment
- Create and maintain a Behavioral Health/SUD registry and input patient data
- Conduct outreach and activities to patients for scheduling appointments, preventative screenings, reminders, and missed opportunities for Behavioral Health Patients
- Conduct intake sessions and assessments for SUD patients
- Assess patient compliance with clinic guidelines
- Provide health education and promote overdose prevention
- Organize and promote opioid-related workshops and training i.e. naloxone administration training for at risk patients and family members
- Conduct outreach to increase community members’ knowledge of resources available and to serve as liaison between Betances resources, community resource programs, and support
- Work with patients and providers to determine what additional educational tools or support is needed;
- Accesses services and situations for both immediate and long-term needs of clients, including:
- Identifying and addressing critical barriers to care, including poverty, housing instability, rent burden, and lack of health insurance, to improve access to behavioral health services by conducting intake assessments such as the Protocol for Responding to and Assessing Patient Assets, Risks, and Experiences (PRAPARE) and the Social Care Network (SCN) Screening tools
- Provide support to patients in overcoming health-related social needs by connecting them with appropriate community resources and services.
- Participate in PRAPARE, Social Care Network and Population Health Management (PHM) team meetings and committee activities.
Salary : $45,000 - $50,000
