The Patient Navigator will provide care coordination services to members with chronic health conditions. Patient Navigator will act as an intermediary between providers and members - serving in numerous roles such as educator, evaluator, service coordinator, community resource researcher and more.
2-years’ experience in the healthcare, community health, social work, supportive housing, or other
social service interventions
Excellent customer service, organizational, interpersonal and communication skills
Ability to multi-task and work under multiple priorities and deadlines in a fast-paced environment
Proficiency with Word and Excel
$19-$20 an hour

- Conduct outreach activities specially to the loss to care patients
- Conduct community outreach visits to patients
- Provide expedited visit to patients for urgent situations such as hospitalization
- Assist in busy Medical Office
- Call, schedule and confirm patient appointments
- Enrolling patients to Essen Health Care’s program
- Assist in achieving fewer emergency room visits and hospitalizations, better coordination of services, and social service navigation
- Conduct new patient’s assessment screenings consistent with the Scope of Service
- Inform patients of Essen services and give them health education materials
- Keep patients informed of progress of scheduled appointments
- Meet with patients in their homes, community sites, Dr. office, and Health Home office
- Work in the Field
2-years’ experience in the healthcare, community health, social work, supportive housing, or other
