Job Summary

Duties
- Deliver comprehensive nursing care to patients in home settings, focusing on individual needs and preferences.
- Administer medications as prescribed and monitor patients for any side effects or complications.
- Assist patients with daily living activities, including bathing, dressing, and mobility.
- Conduct regular health assessments to evaluate patient conditions and report changes to the healthcare team.
- Collaborate with family members and other healthcare professionals to develop and implement personalized care plans.
- Provide emotional support and education to patients and families regarding health management.
- Maintain accurate documentation of patient care activities, including medication administration and vital signs.
- Ensure compliance with safety protocols and infection control measures during all interactions.
Qualifications
- Valid nursing license (LPN or RN) in the state of practice.
- Proven experience in home health care, nursing homes, or hospice care preferred.
- Strong skills in patient care, including the ability to lift and assist patients as needed.
- Knowledge of medication administration procedures and safety protocols.
- Excellent communication skills for effective interaction with patients, families, and healthcare teams.
- Ability to work independently while demonstrating strong decision-making capabilities.
