Overview
We are seeking a compassionate and dedicated Home Care Nurse to provide high-quality care to patients in their homes. The ideal candidate will possess a strong background in home health nursing, with experience in developing care plans and monitoring patient health. This role is essential in ensuring the comfort and well-being of our patients, particularly those requiring dementia care, hospice care, and senior care. As a Home Care Nurse, you will play a vital role in enhancing the quality of life for individuals in need of assistance.

Responsibilities

• Develop and implement individualized care plans tailored to each patient's needs.
• Monitor patients' health status through regular assessments and observations.
• Assist patients with daily living activities, including personal hygiene, mobility, and meal preparation.
• Provide specialized care for patients with dementia or those requiring hospice services.
• Collaborate with family members and other healthcare professionals to ensure comprehensive patient care.
• Document patient progress and report any changes to the healthcare team.
• Lift and transfer patients safely as needed, ensuring their comfort and safety.

Requirements

• Valid nursing license with experience in home health or similar settings.
• Strong knowledge of patient monitoring techniques and care plan development.
• Experience in providing dementia care, senior care, or hospice care is highly desirable.
• Excellent communication skills to effectively interact with patients, families, and healthcare teams.
• Ability to lift patients safely and assist them with mobility as required.

Pay: $14.00 - $16.00 per hour
