Job Summary: The Community Integration Specialist will work closely with adults with developmental disabilities to promote independence, community involvement, and overall well-being. The role involves supporting individuals in a variety of daily activities, helping them integrate into community settings, and fostering skill development to improve their quality of life. This position requires a compassionate, patient, and dedicated individual who is passionate about enhancing the lives of people with developmental disabilities.
Key Responsibilities:
- Community Integration: Support clients in participating in community-based activities and events, such as social outings, volunteering, shopping, or public transportation, ensuring that they feel included and empowered.
- Individualized Support: Assist individuals with daily living skills, including personal care, meal preparation, and hygiene routines, as required. Provide individualized support based on the person-centered care plan.
- Behavioral Support: Implement behavioral strategies and interventions to address challenges and promote positive behaviors. Support clients in achieving goals related to independence and social skills.
- Skill Development: Work with clients on building life skills such as communication, socialization, self-advocacy, and employment readiness. Create learning opportunities that align with the individual's abilities and goals.
- Documentation & Reporting: Maintain accurate records of daily activities, progress toward goals, and any changes in behavior or health. Ensure documentation is consistent with agency policies and regulatory requirements.
- Collaboration: Work with a team of other specialists, family members, and service providers to ensure the best outcomes for individuals. Participate in care plan meetings and provide input regarding client needs and progress.
- Health & Safety: Ensure the safety and well-being of individuals at all times, both in the facility and during community outings. Administer basic first aid or support in emergencies as needed.
Qualifications:
- High school diploma or equivalent (required). A bachelor’s degree in social work, psychology, special education, or a related field is preferred.
- Experience working with individuals with developmental disabilities or in a similar setting.
- Knowledge of community resources and strategies for integrating individuals with disabilities into community life.
- Strong communication and interpersonal skills.
- Ability to be flexible and adapt to changing client needs.
- Ability to work independently and as part of a team.
- CPR/First Aid certification (or willingness to obtain).
- Valid driver’s license and reliable transportation (if applicable).
Preferred Skills:
- Experience with behavior management techniques.
- Familiarity with the use of assistive technology and adaptive equipment.
