```Duties```
- Conduct comprehensive assessments of clients' needs and develop individualized care plans
- Provide case management services to clients, including coordinating and monitoring their access to necessary resources and services
- Collaborate with other professionals, such as social workers, counselors, and healthcare providers, to ensure holistic care for clients
- Advocate for clients' rights and ensure they receive appropriate support and services
- Monitor clients' progress and make necessary adjustments to their care plans as needed
- Document all client interactions, assessments, and interventions accurately and in a timely manner

```Experience```
- Bachelor's degree in social work or a related field (Master's degree preferred)
- Previous experience in case management or a related field
- Knowledge of Medicare regulations and eligibility criteria
- Familiarity with crisis intervention techniques and crisis management protocols
- Experience in addiction counseling or behavioral health is highly desirable
- Proficiency in motivational interviewing techniques
- Understanding of child welfare systems and workforce development programs
- Strong organizational skills and the ability to prioritize tasks effectively
- Excellent communication and interpersonal skills
- Ability to work independently as well as part of a multidisciplinary team

Job Type: Full-time

Pay: $21.00 - $25.00 per hour

Expected hours: 20 – 50 per week
