Community Case Manager- UCDVS

POSITION SUMMARY:  The primary role of the Community Case Manager is to provide services that are designed to reduce risks facing victim/survivors who have been assessed through the program’s LAP to be at high risk of homicide.  The position will initiate outreach and provide services to the target population through the program’s catchment area.  The position will provide trauma informed services, empowering victims with respect to their safety and right to self-determination

 

The required days for this position are Sunday- Thursday from 12-8pm 

This position will be working collaboratively with three local police departments in the greater New Haven area. 

 

Accountable and responsible for the following results 

Direct Services to victims of domestic violence

  • Provide trauma informed counseling/ case management services, maintain up to date case records, initiate referrals, and work pro-actively with survivors of violence to address and enhance their safety needs and those of their families;
  • Link parents and children with existing community resources to address the violence and improve the safety of the children and non-abusing parent, such as, health clinics, mental health services, housing information, public transportation, legal resources, immigration, food and clothing, welfare offices, local school information, youth services, child care, job training and employment opportunities.
  • Provide child development training, parent education and safety planning on behalf of children
  • Work collaboratively with clients to assist in updating their safety plans for short and long-term planning;
  • Provide clients with information and referral to resources outside the domestic violence program;
  • Accompany law enforcement to collaborate with client on safety needs when appropriate;
  • Assist with transportation for sheltering and case management needs of clients

Program Implementation/Evaluation

  • Develop a service plan for each family utilizing the Danger Assessment and VIGOR assessment;
  • Perform assignments as directed by Program Manager to fully implement the grant project activities, initiatives, and intervention programs;
  • Engage collaboratively with ASC personnel to fully implement the grant project activities, initiatives, and intervention programs;
  • Distribute and collect client survey for each client receiving in-person services
  • Complete accurate statistical and narrative reports and submit them in a timely fashion
  • Attends training sessions, workshops, seminars as requested by supervisor, or as needed for professional development, or as required for accreditation or licensing.
  • Required to use own vehicle on company business or to transport clients
  • Regular and consistent attendance is an essential function of this position
  • May perform other assignments and duties as assigned.

Qualifications


MINIMUM QUALIFICATIONS:

  • Bachelors Degree required.
  • Bi-lingual (English/Spanish) preferred.
  • 2 years Advocacy and/or direct service experience working with survivors of domestic violence in a community-based setting preferred.
  • Experience assisting traditionally underserved populations with a developed understanding of issues of diversity essential.
  • Crisis intervention and counseling skills.
  • Knowledge of community resources.
  • Must be proficient in Microsoft 365. Must be able to navigate through electronic health records system (EHR).
  • Must be able to attend required training during first week of employment, which is Monday through Friday, 8:15 a.m. to 4:30 p.m.
  • Must be able to obtain and maintain the domestic violence counselor certification per CGS 52-146K before providing direct service to clients, followed with six-hour recertification yearly.
  • Must have and maintain a valid driver’s license.  Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.
