SUMMARY
The Case Manager provides case management services to young and adult individuals living in the community, playing a key role on the treatment/recovery team. Responsibilities include implementing clinical supports, skill-building, and daily living assistance to promote independent community living. The Case Manager manages a caseload, oversees support services, and supervises paraprofessional staff who teach essential community integration and daily living skills. This position may be part-time and combined with other part-time roles, such as paraprofessional duties.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Develop and implement individual support plans for clients in the community.
Collaborate with DMHAS clinicians and team members on client goals.
Coordinate care, fill in for other case managers as needed, and ensure medical health issues are addressed, including scheduling appointments, providing transportation, following doctors’ orders, and communicating health concerns to the supervisor.
Monitor and assist clients with self-medication.
Guide clients in developing community living and daily life skills, such as hygiene, budgeting, cooking, and decision-making.
Supervise support staff, implementing behavioral plans, teaching strategies, and monitoring programs.
Assist with completing various assessments, including safety, behavioral, life skills, and risk assessments.
Act as an essential employee, aiding in staff scheduling to meet service requirements.
Safely operate motor vehicles, including personal vehicles, for work duties.
SUPERVISORY RESPONSIBILITIES
While the role doesn’t include direct reports, it involves overseeing the implementation of individual plans, ensuring adherence to behavior and life skill plans, reviewing documentation, and monitoring appointments. The Case Manager may also assist with scheduling recovery assistants.
EDUCATION and/or EXPERIENCE
Bachelor’s degree in a related field with relevant clinical experience; an Associate’s degree with three years of experience may be acceptable. A Master’s degree and supervisory experience are preferred.
