The Case Management Discharge Scheduler ensures a seamless transition for patients from the hospital to home or other care settings by facilitating timely and effective post-acute follow-up appointments. This role involves collaborating with healthcare providers, patients, and families to coordinate care plans, address barriers to follow-up, and promote continuity of care. By aligning discharge processes with individual patient needs, the Discharge Scheduler plays a pivotal role in optimizing recovery, reducing readmissions, and enhancing overall patient satisfaction. This position directly contributes to the organization’s mission of delivering exceptional care and achieving positive health outcomes.

Qualifications:

• High school diploma/equivalent
• 1 year of Discharge planning/Case Management experience

• Previous clerical experience and interest in case management, healthcare coordination, or patient advocacy is a plus
